NHS Central London, Hammersmith and Fulham and West London Clinical Commissioning Groups (CCGs) would be grateful if you would assist them by completing the 111 patient feedback questionnaire. This questionnaire has been devised to help improve the 111 service provided locally by London Central West (LCW) for patients.

This questionnaire will take just a couple of minutes to tell them of your experience using the 111 service. Your identity will be protected and your views remain confidential. The information gathered from this questionnaire will be used to improve the service in the future.

If you have used the LCW 111 service previously complete the entire questionnaire; if you have not, complete Section 1 only.
